Use this form to request financial assistance for support with Girl Scout program fees, camp registration deposit, uniform, and/or program material costs.

No application is needed for membership dues assistance; select "Financial Assistance" as your payment method in My GS when you join or renew.

If requesting financial assistance for Girl Scout youth, this form must be completed by the Girl Scout's primary caregiver. If requesting assistance as a Girl Scout volunteer, the volunteer should complete this form on their own behalf.
