502 GSUSA Fall National Awards Nomination Form

This JotForm has replaced the paper form #502 for all volunteer awards presented at the council-wide National Awards Ceremony in the fall. The awards listed below are for service performed between October 1, 2025, and August 31, 2026.

Nomination Submission Details:

Submission Window: May 1 - August 31
- Complete and submit this form with attached required letters of endorsement for each candidate during this window
Volunteers who received Spring Council Recognitions are still eligible for National Awards for that same service if it meets the award criteria
Letters are welcome from anyone who has been touched by the volunteer’s service - families, youth, volunteers, young, old, and everyone in between!

Ceremony Details:

Thursday, November 12, 6:30-9:00 p.m.
- Council-Wide Zoom Ceremony
Awardees will be invited to the ceremony and notified that they are receiving an award in mid-October
Nominators will also be notified that their nominee is receiving an award in mid-October
